User focused monitoring survey

Friend's User Focused Monitoring Team is conducting a survey looking into the Community Mental Health Teams at Windmill House in Clevedon and The Coast Resource Centre in Weston-super-Mare. This survey is aimed at people who have received a service from either of these teams.
